Lost or Stolen ID Badges
Lost or stolen badges are a security emergency, and they must be reported immediately:
- If lost or stolen during normal business hours (Monday – Friday, 6am-6pm):
- If your PIV/RLA is STOLEN, report it to the NIH Access Control at (301) 451-4766 (or email Facility Access Control at FacilityAccessControl@mail.nih.gov), the police, your Administrative Officer (AO) and your supervisor immediately, within 1 hour of discovering it is stolen.
- If your PIV is LOST, report it to the NIH Access Control at (301) 451-4766, your AO and supervisor within 1 hour of discovering it is missing.
- If lost or stolen outside of normal business hours (Monday – Friday, 6am-6pm):
- Stolen badges must be reported to your local police department to have a police report filed regardless of when it occurs.
- Email Facility Access Control at FacilityAccessControl@mail.nih.gov as soon as possible and call 301-451-4766 during next available business hours to ensure the badge access has been disabled.
- Report lost/stolen badges to your AO and supervisor as soon as possible during the next business day.
Your badge will be disabled so no one can use it, and your Administrative Officer (AO) will need to report your badge as lost/stolen in the NIH Enterprise Directory (NED) to authorize a replacement ID badge. You will need to bring in one form of ID from the Approved Identifications Document to your appointment when picking up your replacement ID badge.
If your ID Badge is broken, contact the NIH Division of Personnel Security (DPS) Helpdesk at (301) 402-9755. They can set up an appointment for you to receive a replacement badge.
Broken Badges -- Start with Access Control
Badge holders working at the Bethesda campus or at nearby locations should bring their broken badge to one of the following DPS Badge Issuance Centers:
- NIH Bethesda Main Campus - Building 31, Room 1B03 (Main Office)
- NIH Bethesda Main Campus - Building 10 (Clinical Research Center), Room 1C52 (South Lobby)
- Shady Grove Campus - 9609 Medical Center Drive, Rockville, MD
(SG Terrace Level, NIH Badging, Room #513/515) - Bayview Biomedical Research Center (BRC) - 251 Bay View Boulevard, Baltimore, MD
- National Institute of Allergy and Infectious Diseases (NIAID) - 5601 Fishers Lane, Rockville, MD

Troubleshooting Guide to Diagnose Problems with Your Badge
1. Were you able to get onto campus and/or any buildings/rooms with your badge today?
2. Were you able to log into your computer and/or any IT systems (e.g. NED) with your HHS ID Badge and PIN?
- If YES, your badge should be working correctly. Please contact the NIH DPS Helpdesk at (301) 402-9755 or ORSpersonnelsecurity@mail.nih.gov or submit a ticket to the CIT IT Support if you have any concerns.
- If NO, please proceed to question 3.
3. Did you try logging in with your HHS ID Badge on someone else’s computer?
- If YES, please proceed to question 4.
- If NO, please try logging in with your HHS ID Badge on another computer. Then proceed to question 4.
4. Were you able to successfully log in using someone else’s computer?
- If YES, the card reader on your computer is the issue. Please contact the NIH DPS Helpdesk at (301) 402-9755 or visit CIT IT Support.
- If NO, your HHS ID Badge is the issue. Please contact the NIH DPS Helpdesk at (301) 402-9755 or ORSpersonnelsecurity@mail.nih.gov.
