USCIS Form AR-11/Change of Address
The U.S. Citizenship and Immigration Services (USCIS), Department of Homeland Security (DHS), must be informed of all address changes within 10 days of the move.
It is your responsibility, and not the responsibility of the Division of International Services (DIS), to comply with this requirement in a timely manner. Failure to report the change of address is a violation of immigration status and may lead to termination of your stay in the U.S. and/or other penalties as set by the DHS.
NIH Scientists: It is important that you also notify the DIS of your address change by completing our
Change of Address Request in the DIS Customer Portal.
FDA Scientists: Please review the FDA Change of Address webpage. It is important that you also notify the DIS of your address change by completing our Change of Address Form and emailing it to our attention at
DISFDA@mail.nih.gov.
Also provide your new address to your Institute/Center’s Administrative office (and Office of Human Resources for those in employee/ FTE designations).
Most individuals in FTE designations must complete the Form AR-11 to notify USCIS of a change of address. Examples of such individuals are those holding H-1B, O-1, or TN status. Click

Notes:
- If you have an application pending at a USCIS Service Center or District Office, a separate address notification is required. Follow the directions on the
USCIS web site.
