The U.S. Citizenship and Immigration Services (USCIS), Department of Homeland Security (DHS), must be informed of all address changes within 10 days of the move. It is your responsibility, and not the responsibility of the Division of International Services (DIS), to comply with this requirement in a timely manner. Failure to report the change of address is a violation of immigration status and may lead to termination of your stay in the U.S. and/or other penalties as set by the DHS.
It is important that you also notify DIS of your address change by completing our Change of Address Form and emailing it to our attention at DISFDA@mail.nih.gov.
Please also provide your new address to your Center's Administrative Office and the Office of Human Resources.
Most individuals in FTE designations must complete Form AR-11 to notify USCIS of a change of address. Examples of such individuals are those holding H-1B, O-1, or TN status. Click here to report your address to USCIS.
F-1/J-1 students however should contact their sponsor within ten days of the change for the sponsor to update their new address in SEVIS.

- If you have an application pending at a USCIS Service Center or District Office, a separate address notification is required. Follow the directions on the USCIS web site.
