Accompanied Child Visitor Form

NIH employees can use this form when they bring children onto campus under their supervision.  The form is presented to the guards at an employee entrance to campus and the Children are issued visitors badges.

After Hours Work Permit

Contractors must apply to work on campus after hours; 6pm to 6am on normal business days and 24 hours on weekends and holidays.  The form must be completed listing every room or facility the contractor will access and list the work to be done and submitted to the Division of Police.

Explosive Powder and/or Power Actuated Tools

Contractors using explosive powder or power actuated tools must advise the Division of Police in advance using this form.

Motor Vehicle Accident Report (NIH vehicles)

Employees involved in an accident while driving a fleet vehicle must use this form to report the incident to their supervisor.

Exchange of Information (personal vehicles)

This form covers everything you need for an insurance claim after a minor property damage vehicle collision and satisfies the State of Maryland legal obligation for parties to exchange information after the accident.
