Handicap Parking Request

Handicap Parking Request

Employees who would like to request a handicap parking permit should fax a Handicap Parking Application to OMS at (301) 402-0673.

Parts A and B of this form must be completed before faxing. Part C will be completed by OMS.
A note from the employee's personal health care practitioner may be attached in lieu of completing part B. However, a note is not required.
