If missing information exists, a case manager will reach out to you using your email address listed in the NIH Enterprise Directory (NED). Missing information will delay the badging process. If your information or attachments are incomplete or inconsistent, you will be contacted to make corrections to your form.
Regarding Your NIH Badge Enrollment and Fingerprinting:
We may also require you to complete your PIV Badge Enrollment and/or Fingerprinting in order to submit your investigation request. You will need to complete this requirement as soon as possible to avoid delays in submitting your investigation request and thus, being authorized for your NIH PIV Badge.
In the event you are contacted to make corrections, you will need to:
- Log into eApp
- Review the details of the request and make corrections via the eApp system
- Answer all Yes/No questions again
- Re-certify and digitally sign your eApp questionnaire (you may also save an archival copy at this step)
- Re-release your eApp to the agency
Comply with your DCSA Investigator:
Once the investigation has been scheduled, you may be contacted by an investigator for questions or to schedule an interview as part of the investigation process. It is critical that you comply with requests from your investigator. Failure to do so can result in your investigation being cancelled which can result in revocation of your NIH badge (if you were issued one). For the interview, you will be required to provide photo identification, such as a valid state driver's license. You may be required to provide other documents to verify your identity, as instructed by your investigator.
