Report a National Institutes of Health (NIH) Fire Safety Issue or Concern


NIH Community Fire Safety Complaint Report

Please complete the following form and select the submit button to notify the NIH Division of the Fire Marshal, Fire Prevention Services Team, of a fire code violation or fire safety issue. All reports will remain anonymous.

Providing personal information is not necessary, However, if you would like to receive an electronic receipt, please provide your e-mail address.
 

Please note: The form below shall be used to report fire safety issues ONLY for National Institutes of Health (NIH) owned facilities on federal campuses in Bethesda, MD, Poolesville, MD, Rocky Mountain Laboratory, MT, Research Triangle Park, NC, and NIH portions of Ft. Detrick, MD. For fire safety issues involving NIH leased facilities, please contact the local fire safety authority having jurisdiction.
